Physician lead services can include ambulatory detox/ withdrawal management and co-occurring medication management support. Physician lead services are designed for clients who have experienced some form of traditional treatment in the past or families that are well supported and whose obligations do not allow them to seek normal standard of care. The client is supervised in their home by an assigned caregiver and receives daily visits by a team member throughout care.
Client and staff safety is our top priority during the in-home process. When you call to speak to one of our care specialists you will complete an assessment to determine your eligibility.
Intensive Case Management is a more involved approach than our Core Program, with 9 - 15 hours weekly of purely case management, alongside services of weekly therapy, and family sessions. We will work with you in your day-to-day, workplace, and home life, and support you every step of the way.
Other services can be added or removed based on client needs.
Personal and professional coaching serves as a lower level of service need. Coaching is designed to set goals, maintain accountability, and grow in the directions you want when you need a push, punch, or feel stuck. Coaching services are tailored to the individual and placed with the most appropriate fit based on goals. In some circumstances a team of two are assigned. Sessions are typically one hour in length and biweekly.
Along the way or for the whole of our lives, we may need the support of medications to let us live a fuller life. Let our team help you with any psychiatry prescriptions and care you need in your healing journey. With our specialized and well-rounded team, you will be understood and vouched for.
We offer resourcing, placement assistance and case management services tailored specifically to support your family law or diversion court needs. We can work with families directly or as retained by attorneys to promote growth aligned with the desired outcomes or mediation. We understand the process and unpredictable outcomes that can and often do occur.
Peer Mentors are knowledgeable professionals who provide support to clients that are experiencing challenges in their early recovery or most precarious stage of recovery. Peer Mentors can help integrate clients back into their homes post-treatment, help maintain and implement their healing /recovery plan and provide extra support during stressful times. Recovery companions are trained are trained in relapse prevention methods, crisis prevention, and coping skills.
Aftercare is similar to coaching, but most often assigned to those in either treatment for mental health, substance use disorder, or co-occurring disorders. Aftercare is a crucial part in support and accountability when the treatment experience ends and "Real Life” resumes to ensure the most probable success.
